    2. 1 X 1 RECENT ID PICTURE (3 PIECES)

    3. ORIGINAL BIRTH CERTIFICATE (PSA CERTIFIED)

    4. CERTIFICATE OF GOOD MORAL CHARACTER

    5. RECENT REPORT CARD

    6. FORM 137 (requested from student's previous school)

    Please place all hard copy documents in a LONG BROWN ENVELOPE, clearly labeled with the student’s name, or e-mail copies to registrar@stsseducation.com.
